A CSB safety video on the October 26, 2019, hydrogen sulfide release at the Aghorn Operating waterflood station in Odessa, Texas. The release fatally injured an Aghorn employee who was working at the facility that evening, as well as his spouse who attempted to locate him at the facility after he did not return home.
I can’t believe that the wife was allowed onto such a property and ended up being killed as well. The children, instead of just losing a father, lost their mother as well. What a tragedy. If I was in this field I would probably have two of those monitoring devices, one in my pocket and one maybe stuck to a part of the dash or mounted somewhere in the vehicle so I could see it.

Where in the world was OSHA or any safety inspection overseer? How many lives have been lost in the oil and chemical industry due to total disregard of basic safety procedures? I worked in the oilfield for eight years and can personally attest that for all the talk about safety, lost time accidents, etc., actual safety took a back seat to profits almost inevitably. Condolences to the families of the deceased.

I learned a simple rule in school chemistry class when working with this gas – if you smell H2S, and then stop smelling it, get the hell out. At dangerous levels, the foul smell sort of goes sweet.

I know the wife didn’t work at the plant, but surely she glanced in the worker’s truck to see if he was there when she arrived? If so, how did she not see or hear the worker’s personal alarm going off on his front seat? Surely that alarm on his front seat, plus his extended absence would have sent danger alarm bells going off in the wife’s mind?
I feel so bad for the kids, imagine the horror of growing up knowing both your parents were killed metres away from you and you were blissfully unaware so couldn’t save them…

Just for the record, a jury returned an indictment on Aghorn and the VP of operations last month on a bunch of charges. Hopefully this kids family will get some justice for their lost love ones.
H2S is nasty stuff.. it kills by inhalation, skin contact, and by being a component in explosive atmospheres…. One breath above a certain concentration can be fatal…

Good video but the woman smiling while talking about this was a little disturbing to me. If no one had died it would be fine but two kids have no parents now so her smiling was a little too much for me.
You can’t get business people to pay attention to safety without a gun to their heads. They never do anything more then is legally required and will use every loophole to " get around " regulations. Voluntary guidelines do not work. If they did, these videos would not exist.
I’m a pump technician and have seen those ceramic plungers completely shatter.
Just yesterday while working on a pump on a wellhead with about 14,000 PPM, my 4-gas monitor went off when I removed a plunger. I dropped the plunger and went to a muster area. My employer is very small but spares no expense when it comes to our safety.

Companies could incorporate an Undercover Boss knockoff and call it Undercover Expert. Experts could job-shadow individuals at different facilities testing their training and knowledge using only hidden cameras on the expert’s person. No camera crew or anything similar to potentially taint the natural behaviors. Afterwards the footage and experience could be examined to evaluate if more training or an update to training is necessary.

I have been out of Texas for a long time, but I remember a fairly toxic, macho culture of contempt for safety equipment. I’m not surprised the dude left his personal detector behind.
The decision to leave his personal monitor in the truck not only killed him, but his wife as well, and orphaned his 2 kids.
it’s sad that in order for this to happen, several mistakes were made in a row. any of them could have broke the chain
if he forgot the detector, sign or audio warning could have reminded it
if not, facility sensors could have alerted him. if not, he should have turned pump off, there could be sign for that
his wife could have educated on dangers in place where her husband worked in
if not, gates should have been closed

I can’t imagine driving to a facility where h2s is known to exist in very high concentrations, having a meter in your truck, and not wearing it into the building. At 1000 ppm, 1-2 breaths causes unconsciousness and death.
It’s a horrible accident, which anyone could make, and I don’t want to blame the victims. The wife coming by likely had no idea how immediate the danger was. Its terrible, two kids lost both their parents in the same day. I guess it’s a awful reminder that safety measures are put there to save lives, and ppe can and will save lives but only when used. We all like to complain about additional equipment and rules, but too often rules are written in blood. Safety equipment does you no good if its sitting in your service truck. This guy probably entered that building a thousand times(and watched old timers do it a thousand times before him) and was fine, but what you don’t know can still hurt you. For the love of god, use every precaution you have. Live to fight another day. Dont let a silly mistake end you, too.
I’m a trucker and recently moved a trailer for a company from one refinery to another in the western US. I had to watch a 20 minute video about H2S just to get on the property. They take it very seriously. It’s a tragedy what happened here.
